Please note that any forms downloaded from this website for submission to the Division of Building & Codes must be printed doubled-sided if the form is more than one page. Forms will only be accepted at the Division of Building & Codes that are printed in this manner.

Application for Building Permit - must be printed double sided for submission
Sidewalk/Barricade Permit Application
Certificate of Appropriateness (to be filled out & submitted if you live in a Historic District) - must be printed double sided for submission
Plot Plan (for exterior work such as a Deck, Fence, Pool or Shed)
Photo Log (photos of the street facing exterior of the structure need to be submitted if you live in a Historic District or an Overlay District)
Paint Chip Form (must be submitted for exterior painting in a Historic District or an Overlay District)
Architect's/Engineer's Letter - must be completed by a NYS Licensed Architect or Engineer if required
Electrical Permit Application (also serves as Alarm Permit Application) - must be submitted to this Office only by a Licensed Electrician - must be printed double sided for submission
Plumbing Permit Application - must be submitted to this Office only by a Licensed Plumber - must be printed double sided for submission
Elevator Permit Application - must be submitted to this Office only by a Licensed Elevator Company representative
